To print the Pedigree Form for Citizenship:

1. Click on the pop out icon on the actual pedigree chart that looks like this:

2. Then choose the print icon that looks like this:

Once you have completed this form, you will use the Genealogy Upload link here to upload the pedigree chart along with all your other documents. PEDIGREE FORM UPLOAD
