The application form below must be completed in its entirety. The application may take 15 to 30 minutes to complete. Please, take your time and answer every question completely to the best of your ability. Remember to Apply and to Pay to complete this step.
A. Fill out the complete online application form. A separate application must be submitted for each individual. Once completed, submit it. You will be emailed with further instructions so be sure to provide complete information. You will then use the PAY for application link to complete the application by paying the application fee online. If you cannot pay online, you must send your payment to the address below. You will not receive further instructions until payment is rendered and completed. Payments are non-reimbursable.
B. PAY ONLINE or Mail payments to: Sac River and White River Bands, 119 White Dove Lane, Thornfield, MO 65762. Make checks payable to: Sac River and White River Bands.

Here is a list of documentation you should gather and have on hand:
Proof of Identity: Provide only two (2) of the following documents. One document must contain a photo of the applicant and the other date of birth.
• *Copy of a valid unexpired State issued driver license or State issued ID card (a driver’s learners permit is acceptable);
• *A certified State issued birth certificate or amended birth certificate if adopted;
• A U.S. military identification card:
• A valid Federal or State government-issued identification card (Law Enforcement, Emergency Services etc.);
• A valid unexpired U.S. Passport;
• A certified Federal, State, or local issued marriage, divorce, and/or change of name document;
• A certificate of live birth or certificate of Baptism if born on an American Indian Reservation or U.S. Military Installation or Embassy overseas.
Proof of Social Security Number: Documents must contain Applicants name and full SSN.
• *Social Security card;
• Print out from the Social Security Office showing full name and SSN;
• W-2; SSA-1099
Proof of Name Change:
• Updated U.S. Passport or Passport Card;
• Court Order for Legal Name Change;
• Marriage Certificate or certified Marriage License;
• Court Adoption Document;
• Divorce Decree.
PHOTOGRAPH REQUIREMENTS: Good quality smart phone selfies are acceptable but must meet the requirements below. If you have a professional Photo you may send it in with your packet.
You must have a driver’s license portrait style photograph taken for all tribal IDs. The following guidelines are required for the photograph:
• Full-face directly facing the camera. The photo must include the crown of the head and below the chin, preferably a couple inches above the head to the top of the shoulders. Photos require a neutral background, white or light blue.
• Both eyes must be fully open.
• Applicant must have a neutral facial expression.
• Only the individual being photographed can be in the photo.
• Plain every day attire, no uniforms or headgear. Hair should be combed or brushed back behind the ears, so they are visible for the photo. The only exception to headgear is if it’s a religious requirement.
• If you wear prescription glasses, and have a restriction on your driver’s license, you should wear your glasses for the photo. Please wear clear lensed glasses, no tinted lenses please. Absolutely no dark sunglasses or mirrored lenses. If you use any other medical devices, such as hearing aids, these may be worn for the photo.
• Hands-free devices, headphones, earbuds, cell phones, or any other similar devices are not acceptable in your photo. Please make sure your photo is as level and straight as possible.
